Purchase Log and P-Card Return Form

Anyone who has made a purchase for a Baker-related activity must fill this out this form in order to submit a receipt of their purchases and return the p-card. If an order on Amazon was placed, both the Purchase Log and Amazon Log forms should be filled out. For those who used a P-card, after they fill out the form, someone will be in contact with you all again in order to pick up the pcard.
